This is an official survey developed by NAPWA for individuals who are testing. If you are agency is providing HIV testing, download these PDF files (both English and Spanish) and distribute them among your clients. After completion of your event, mail the surveys (the filled out survey sheets) to us. |

The following are important instructions to print the PDF version of the
NHTD surveys:
1. You must print from a laser printer. You do not need to print in color.
2. In order for the scanner to collect the data, please purchase 24 lb. paper.
3. When you're on the printing screen, please select the following options:
1. Go to "Page Scaling" and SELECT: NONE
2. UNSELECT the checkbox next to: AUTO-ROTATE and CENTER
3. Both versions are 4 pages long. One ream/packet of paper includes 500 sheets, which can yield 125 surveys when you print. |
